Madhu has lead Paragon Rehabilitation from a small company providing contract therapy services to 6 facilities to a company that now successfully provides contract therapy services to over 130+ partners including skilled nursing facilities, home health providers, hospitals and outpatient settings. His ability to critically analyze financial data, implement growth strategies, utilize his own clinical experience and develop strong teams while providing outstanding customer service has resulted in successful outcomes for our patients as well as our partners. Madhu has developed a strong team culture with expectations of excellence resulting in successful employee retention, continued company growth with proven financial success. Madhu leads with the philosophy that providing ethical, compassionate and quality services to our patients, success will follow. Madhu obtained his PT degree from MGR University in India. He is a member of American Physical Therapy Association (APTA) and National Association for Support for Long Term Care (NASL).

Paula is responsible for overseeing and implementing the operations in the Indiana, Kentucky and Ohio markets. She also assists in the development of facilities in GA, NM, Fla, and SC. She has strong experience within healthcare leadership roles, training and development and fiscal management. She is a patient advocate assisting her teams to achieve positive outcomes thru advanced trainings, in- services and focus on customer service. As a member of our leadership team, she works with other leaders and teams to create and achieve our goals. Paula has a Bachelors degree in Occupational Therapy from Spalding University, as well a Bachelors degree in Economics. She is vital-stim certified and certified in lymphedema management. She specializes in talent and engagement as well developing great working relationships with various vendors. Additionally, Paula is an active member of AOTA and KOTA. Paula also maintains a strong relationship with Spalding University where she has spoken to current students on the role of OT in the community.

Andy is a licensed Physical Therapist and Certified Athletic Trainer. Andy has been in health care management for over 15 years with experience in hospital-based inpatient and outpatient, acute care, long-term care, and private practice. Andy utilizes a strategic approach in his efforts to promote rehabilitation services for Paragon Rehabilitation partnerships. The use of data analytics and current trends in healthcare are critical in these efforts. The data and trending are analyzed to provide a platform for Paragon Rehabilitation to promote improved outcomes and patient satisfaction leading to financial success for our partners. He is an active member of the APTA, and state and national healthcare associations. Andy strives to promote individual accountability and ownership for leaders to whom he provides support. He believes quality care is a combination of great customer service, proactive communication, and state-of-the-art clinical care. Andy’s roll with Paragon includes both operational and new business support for regional leaders.

Scott Shelton has worked in the healthcare industry for over 10 years. He’s enjoyed numerous positions ranging from inpatient rehab to acute care to SNF/LTC. He began his career with Paragon in 2010 as a staff therapist, transitioned into a Program Director role in 2011, and was promoted to Area Manager in 2014 and then Regional Director of Operations in 2017 where he supports our south region. He says his main goal is to provide compassionate care to Residents while supporting employees and customers at the same time.

Adnan has worked in the healthcare industry for over 10 years. He’s enjoyed numerous positions ranging from inpatient rehab to acute care to SNF/LTC, and has had multiple years of management experience in a variety of skilled nursing and rehabilitation facilities. He began his career with Paragon in 2015 as a Program Director for a very large Trilogy health campus, and was promoted to Area Manager shortly thereafter where he supports both the Michigan and Northern Ohio market. His main goal is to ensure that our patients are receiving optimum rehabilitation services while ensuring employee and customer satisfaction.

Amanda graduated from Lutheran College of Health Profession in 1997 with Associate’s Degree as a physical therapist assistant, and obtained a Bachelor’s Degree with concentration in healthcare management from Indiana-Purdue at Fort Wayne in 2003. Amanda has been a Licensed PTA for 16 years, with experience in acute care, acute rehab and skilled nursing facilities, including 8 years of department management experience. Amanda has a commitment to ensure quality patient care and excellent customer service. She is an active member of the American Physical Therapy Association – Geriatric and Health Administration Section Member.

Denise is a physical therapist graduate of the University of Evansville and has had 20 years of experience as a clinician and 18 years in management. She was the owner of her own private practice, specializing in Orthopedics, Geriatrics and Ergonomic Consulting. Denise is a Wound Care Certified clinician by the National Alliance for Wound Care. She does wound consulting and teaches appropriate wound care to therapists and nurses. Denise researches and develops clinical education programs in all areas of treatment for Paragon. She specializes in the accreditation of those programs and training our Paragon clinicians. This ensures Paragon therapists will deliver state-of-the-art treatments to our patients. Denise is an active member of the APTA, Geriatrics and CEMS Sections.

Gary began his career in healthcare in 2010 specializing in outpatient and long-term care. He joined Paragon in 2012 as a staff therapist, and transitioned to a Program Director role in 2014.  In 2017 he was promoted to Area Manager supporting campuses in Indiana and Ohio. Through Gary’s strong background skills in business, he is dedicated to serving and leading his region to provide exceptional clinical care.

Jill has been in long term care her entire career and has a strong background in home health. She believes strongly in having values, relationship and communications to all be the key to a successful program. She graduated from Rhodes State College in 2010 with an Associate of Science degree, and then decided to return to school to obtain a bachelors in healthcare administration from The University of Northwestern Ohio in 2018. Jill has been in therapy management for for 6 years and is LSVT BIG certified since 2013. She joined Paragon Rehab in 2014 as a Program Director and was promoted in 2018 to the Area Manager for Northern and Central Ohio. She also oversees home health contracts in multiple campuses.

Lisa received her Bachelor’s degree in Speech and Hearing Sciences from Purdue University. Lisa went on to receive her Master ’s Degree in Speech and Language Pathology from Ball State University. Lisa has vast experience with First Steps, school based therapy, Home Health Care, Inpatient Rehabilitation, and Sub-Acute Rehabilitation. Lisa began her career at Paragon Rehabilitation in 2011, with the past three years spent in management. Lisa is an active member of American Speech and Hearing Association (ASHA) and special interest groups: dysphagia and cognitive rehabilitation. Lisa works hard to lead by example and provide outstanding customer service at all times.

Michelle has been a PT for 28 years and worked in an acute rehab setting as a staff PT before being promoted to management positions. She eventually became an Inpatient Clinical Manager prior to transitioning to the SNF setting 21 years ago. She was a Rehab Director in SNF settings for 18 years prior to her hire with Paragon in 2014 where she was promoted to the Area Manager role in 2017. She enjoys working to develop good communication systems and relationships both within and outside of the rehab departments to achieve optimal patient outcomes.

Resident of Latty, Ohio, He has 16 years of experience in long-term care, with 15 years of skilled nursing facility management experience. Mike places a strong emphasis on delivering outstanding customer service to both the residents and facility staff.

Tammy graduated from Vincennes University in 1988. She has experience providing physical therapy in acute hospital, onsite industrial/workcomp, outpatient neuro/orthopedic and long-term care. She started her career with Paragon in 2010 as a Program Director. She was promoted to Area Manager in 2012. Tammy feels it is important to provide excellent customer service with both residents and facility staff in mind.

Becca Keene has worked in the healthcare industry for over 10 years. She received her Masters of Science in Occupational Therapy from Spalding University and has been working primarily in the SNF/LTC setting. Becca has been with Paragon since 2006 and was promoted to Program Director in 2014. In 2017 Becca was promoted to senior PD and in 2018 became an area manager, serving campuses in southern IN and KY. Becca strives to make sure our resident’s needs are the # 1 priority while creating a fun, family type work atmosphere for employees and customers.

Mindy leads Paragon Rehabilitation’s clinical programs, clinical policy development and quality assurance efforts. Mindy reviews clinical analytics and works with Paragon operation teams to strategically develop individualized quality clinical programs to achieve successful outcomes for our patients while meeting the specific needs of our partners. Mindy has extensive experience in healthcare policy, reimbursement and regulatory concerns that affect the post-acute, out-patient and home health industry. She is an active member of National Association for the Support of Long Term Care (NASL) and serves on the NASL Board of Directors. She is a member of The American Occupational Therapy Association (AOTA), Indiana Occupational Therapy Association (IOTA) and the American Association of Nurse Assessment Coordination (AANAC). Mindy is a registered with the National Board for Occupational Therapist (NBCOT) and is certified as a Resident Assessment Coordinator. She received her degree in Occupational Therapy from Indiana University where she also serves on the Board of Directors for the School of Health and Rehabilitation Sciences Alumni.

Marilyn received her Bachelor’s degree from Purdue University, and her Master of Science in Occupational Therapy from the University of Indianapolis. With over 20 years of experience as a clinician, she has provided patient care and management in a variety of settings, including acute care, outpatient, private practice, and long-term care. She has been employed with Paragon Rehab for over ten years, first as a Program Director, and then in Clinical Support beginning in 2010. Marilyn’s expertise includes successful medical review outcomes and appeals management, as well as clinical education focusing on quality documentation and Medicare regulations for Paragon’s team members and customers.

Francis received his Bachelor of Science Degree in Physical Therapy at De La Salle University-Health Science Institute (DLSU-HSI) in 1992. He spent 4 years at DLSU-HSI as Physical Therapy Instructor. His 23-year career as a Physical Therapist has included management and patient care in long-term care, out-patient, home health care, acute care and psychiatric hospitals. Francis began his career with Paragon as Program Director in 2011, and was promoted in 2014 as Clinical Support Specialist where he completes documentation audits for a proactive approach toward Medical Reviews and ensures compliance with Medicare regulation. He is a member of the American Physical Therapy Association.

He spent 10 years in the academe in different capacities as a Physical Therapy Instructor, Clinical Coordinator, Review Coordinator and as Chairman of the Department of PT of the Medical Colleges of Northern Philippines. He was a Program Director for more than 3 years prior to his promotion to Clinical Support Specialist with Paragon Rehabilitation in April 2015.

Erin has worked in skilled nursing and long term care for 16 years, with the last 13 years spent in management at the facility level. Erin joined the clinical support team in May 2015. After graduating from Eastern Michigan University with a Bachelor’s of Science Degree in Occupational Therapy, Erin has always enjoyed working with the geriatric population. Her efforts were directed toward returning each individual to his/her prior level of function, while incorporating humor and fun to improve quality of life. She looks forward to working hard to assure compliance with documentation, to educate our therapists of Medicare regulations, and to share her clinical knowledge.

Kathleen is a speech-language pathologist (SLP) who received her bachelor’s degree from Butler University and a Master’s of Science in Speech-Language Pathology from Purdue University.  Kathleen has worked in the public school system and the home health setting, with a primary focus in skilled nursing and long-term care for over 15 years.  She feels it is a great privilege to serve the geriatric population.  Kathleen is a member of the American Speech Language Hearing Association and has represented Paragon by presenting for the Indiana Speech Language Hearing Association, of which she is also a member. Kathleen has several years of various management experience at the facility level.  She started with Paragon in 2012 as a staff SLP and transitioned to the Clinical Support team in September 2016, working to support other therapists by providing education in therapy clinical documentation, Medicare compliance, and program development.

Jennifer Kima joined Paragon’s recruiting team in 2008. Prior to her move to Paragon, Jennifer served as a recruiter for Healthcare Therapy Services for 7 years. She is responsible for recruiting in IN, KY and IL, and supports therapy recruiting in all Paragon states. Jennifer handles candidate sourcing, pay rate negotiation and paperwork for all new hires, as well as third party contract negotiation. She graduated from St. Joseph’s University in Philadelphia, PA with a food marketing degree.

In September of 2015, Amy transitioned to a recruiting role in an effort to grow in a different capacity with Paragon. By applying the same skill sets she used in business development, Amy strives to obtain new employees that possess qualities such as integrity, dedication, compassion and respect. Chris comes to us with extensive sales knowledge in both long-term care and the hospital industry. He became a Registered Respiratory Therapist in 1988 and started his sales journey in durable medical equipment with Lincare in 1992. He took the exciting leap to Hill-Rom in 1999 as their long-term care sales manager, selling beds and furniture needs for long term care facilities. In 2000 he sold long term care pharmacy services with both Omnicare and Kindred Pharmacy. In 2003, with his enthusiasm and passion, he began promoting safe patient handling equipment to both long term care facilities and hospitals, starting with only 30 customers he grew business for EZ Way, Inc. to over 350 customers to the present day.  Chris is excited about his next sales adventure here at Paragon.
